Continuity of Care (CoC) is a comprehensive and continuous midwifery care approach aimed at improving maternal and neonatal health outcomes through integrated services from pregnancy to family planning. This case study aimed to describe the implementation of CoC for Mrs. Y, a 40-year-old G2P1A0 woman, at Endah Tri A. Private Midwifery Practice, Sidoarjo. Care was provided from December 9, 2025, to February 8, 2026, using the midwifery management approach with SOAP documentation. The services included one antenatal visit, normal delivery, four postpartum visits, three neonatal visits, and one family planning visit with intrauterine device (IUD) insertion. The mother and newborn remained in good condition without complications. Continuous care enabled early detection of risk factors, regular monitoring of maternal and neonatal health, and appropriate postpartum family planning, supporting the quality of comprehensive midwifery care.
