Continuity of Care (COC) refers to continuous midwifery care spanning the third trimester of pregnancy, childbirth, the postpartum period, and neonatal care through to family planning, aimed at enhancing service quality and maternal-infant safety, as well as supporting the reduction of maternal and infant mortality rates. A case study involving Mrs. N 31 years old G3P1001Ab100 was conducted from November 8 to December 11, 2025, utilizing the COC approach and SOAP documentation. Pregnancy related complaints were successfully managed, the delivery proceeded normally, a healthy male infant was born 3,200 g, 51 cm, and the postpartum and neonatal periods were free of complications; the mother opted for the 3 month injectable contraceptive. No discrepancies were found between theory and practice. The implementation of COC facilitates early detection and appropriate management while safeguarding maternal and infant health; therefore, it should continue to be applied in accordance with midwifery service standards.
