The aim of this study was to describe the implementation of Continuity of Care (CoC) in a pregnant woman with mild anemia through comprehensive midwifery care. This study employed a case study design using the SOAP documentation approach. The subject was Mrs. F, a 25-year-old multigravida (G2P1A0) diagnosed with mild anemia (Hb 10.9 g/dL), who received continuous care from 37–38 weeks of pregnancy through childbirth, postpartum, neonatal, and family planning services at TPMB Bidan Ikhdaniatul Khusnia, Pasuruan Regency, from October to December 2025. The results showed that comprehensive midwifery care was successfully implemented throughout all stages. Deep breathing techniques reduced maternal anxiety, spontaneous vaginal delivery resulted in a healthy newborn weighing 3,200 g with Apgar scores of 8–9, and oxytocin massage supported breast milk production during the postpartum period. The mother experienced normal recovery and chose a three-month injectable contraceptive. In conclusion, Continuity of Care effectively maintained maternal and neonatal health, promoted successful breastfeeding, and ensured continuity of family planning services.
