Maternal and infant health remains an important indicator of the quality of health services. Continuity of Care (CoC) is a comprehensive midwifery care approach that provides continuous services from pregnancy, childbirth, postpartum, newborn care, to family planning. This study aimed to describe the implementation of Continuity of Care midwifery care for Mrs. R, 29 years old, G2P1A0, at Jatirejo Community Health Center, Mojokerto Regency. A descriptive case study design was used with a Continuity of Care approach. Data were collected through interviews, observations, physical examinations, documentation, and medical record reviews. Midwifery care was documented using the SOAP method and analyzed descriptively by comparing assessment findings and care outcomes with midwifery care standards and relevant literature. The results showed that continuous midwifery care was provided from the third trimester of pregnancy through childbirth, postpartum, newborn care, and family planning. The pregnancy and childbirth processes proceeded physiologically. The newborn was born healthy, and postpartum and neonatal adaptation progressed normally. Complementary interventions, including endorphin massage, relaxation techniques, aromatherapy, effleurage massage, oxytocin massage, herbal nutrition education, and Kangaroo Mother Care, were provided according to the client's needs. The mother and infant remained in physiological conditions without complications. Continuity of Care integrated with appropriate complementary interventions can support maternal comfort, breastfeeding, postpartum recovery, and optimal maternal and neonatal outcomes.
