Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) remain key indicators of health development. This study aimed to describe the implementation of Continuity of Care (CoC) midwifery services for Mrs. W from the third trimester of pregnancy to family planning at Jatirejo Community Health Center, Mojokerto Regency. A descriptive case study design with a Continuity of Care approach was used. Data were collected through interviews, observations, physical examinations, documentation, and medical record reviews, with care documented using the SOAP method. The results showed that comprehensive midwifery care was successfully provided throughout pregnancy, childbirth, postpartum, neonatal care, and family planning. Complementary interventions helped manage physiological discomforts during pregnancy, while labor, postpartum recovery, neonatal adaptation, and family planning proceeded without complications. Continuity of Care supports comprehensive and continuous midwifery services, contributing to improved maternal and neonatal outcomes.
